Job summary

Reserve Care is seeking a Care Navigation Nurse (LPN) to support referral intake, care coordination, and initial clinical triage, ensuring timely access to hospice services. The role emphasizes one-call resolution and smooth progression of referrals through dispatch.

The LPN must hold an Ohio license, with hospice experience preferred, and have strong communication skills to support patients, families, and healthcare teams in distress.

Qualifications

• LPN licensed in Ohio with hospice experience preferred.
• Excellent verbal and written communication.

Responsibilities

• Retrieve, monitor, and process hospice referrals from multiple sources.
• Learn and use dispatch to schedule appointments for prospective patients.
• Follow up on referrals not progressing to dispatch.
• Perform direct admits to inpatient care centers as needed.
• Answer clinical calls from patients and caregivers and escalate as needed.
• Notify primary care teams of addressed clinical issues.
• Participate in quality and process improvement initiatives.
• Perform other duties as assigned.

The Care Navigation Nurse LPN supports referral intake, care coordination, and initial clinical triage to ensure timely, accurate, and efficient access to hospice services. The role emphasizes one-call resolution whenever possible and seamless progression of referrals through dispatch.

WHAT YOU WILL DO:

• Retrieve, monitor, and process referrals for hospice services and Western Reserve Navigator potential patients from multiple sources, including:
  • CarePort
  • Electronic fax inbox
  • Phone referrals
  • Other designated referral sources
• Learn and utilize the dispatch process to schedule appointments for prospective patients.
• Follow up on referrals that do not progress to dispatch in a timely manner.
• Perform direct admits to inpatient care centers as needed.
• Take clinical calls from patients and caregivers; attempt resolution and elevate appropriately.
• Notify primary care teams of any clinical issues addressed.
• Participate in quality improvement and process improvement initiatives.
• Perform other duties as assigned.

SUCCESS CAPABILITIES:

• Licensed Practical Nurse (LPN) in the state of Ohio
• Hospice experience strongly preferred
• Excellent verbal and written communication skills
• Ability to communicate calmly and compassionately with patients and families in distress
• Strong teamwork and collaboration skills
• Comfortable using multiple technologies, including:
  • Electronic Medical Records (EMR)
  • Phone systems
  • Referral and intake platforms

PHYSICAL REQUIREMENTS:

This is sedentary work requiring the exertion of up to ten (10) pounds of force occasionally and some light work and lifting requiring the exertion of up to 20 pounds occasionally; work requires sitting, stooping, bending, kneeling, reaching, standing, fingering, grasping and repetitive motions; vocal communication; hearing, and visual acuity. Employee is not subject to adverse environmental conditions. This position has the potential to be exposed to environmental elements, internal and external, such as infectious disease, bloodborne pathogens, and chemical and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations.

CONDITIONS OF EMPLOYMENT:

• Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
• Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed

• Ability to provide proof of eligibility to work in the United States.
