Ibis Healthcare is seeking a Case Manager for Women's Residential Services in Florida to provide case management for residents with chemical dependency or co-occurring disorders. • Requires individuals to be at least 21 years of age or older to be a listed driver with the organization, as well as valid Florida driver's license and current vehicle insurance coverage.

Responsibilities

• Conduct client intake, screening, assessment, orientation, and related reporting.
• Develop and implement case management plans addressing housing, employment, engagement in behavioral health recovery, and linkage to ancillary services.
• Develop transition and discharge plans with clients including behavioral health provisions.
• Coach clients on treatment plans and self-help recovery services, health goals, and accountability.
• Facilitate life skills groups with fidelity to evidence-based practices.
• Prepare and maintain clinical records per policy and procedure.
• Interface positively with community partners.
• Participate in client case reviews and team meetings.
• Make referrals to appropriate resource agencies and document outcomes.

• Facilitate life skills group with fidelity to evidence‑based practices.
• Prepare and maintain clinical records in accordance with policy and procedure.
• Interface positively with community partners.
• Participate in client case reviews and team meetings.
• Make referrals to appropriate resource agencies and document outcomes of referrals.
• Provide crisis intervention when needed.
• Assist Behavioral Health Techs in transporting clients as required.
