Description

Monogram Health is looking for a skilled Social Worker eager to make a difference in patients' lives. The Care Manager Social Worker is a key member of an integrated Care Team including a Nurse Care Manager and an Advanced Practice Provider. The patients we serve often face multiple serious diseases and behavioral health challenges. Social workers help remove economic and behavioral barriers to improve health outcomes.

Your Impact

The care team works with patients face-to-face, over the phone, and via telehealth to identify and address social determinants of health. The goal is to build social support networks, navigate behavioral challenges, and support patients through traumatic diagnoses and life-changing diseases. Our mission is to prioritize patient well-being and quality of life, shifting away from processes that often sideline the patient.

Roles and Responsibilities
  • Conduct in-home and telehealth visits to assess social and behavioral status
  • Collaborate with the Care Team for optimal outcomes
  • Assess social determinants of health and develop intervention plans
  • Establish relationships with local Community-Based Organizations
  • Educate patients on resources, facilitate referrals, and follow up
  • Serve as a social determinants subject matter expert within the team
  • Perform behavioral, environmental, and social support assessments
  • Provide education on managing chronic illness to individuals, families, and groups
  • Involve family and social support networks in care
  • Refer patients to behavioral health specialists as needed
  • Assist patients in understanding and adhering to medical and lifestyle recommendations
  • Document patient progress in the care management platform
Position Requirements
  • Telephonic visits with some travel to patient homes
  • Basic Life Support (BLS) certification; company supports certification process
  • Current licensure as an LCSW or LMSW in the posted state
  • Master’s degree in social work and passing of the ASWB exam
  • Potential for minimal domestic travel to Brentwood, TN
  • Ability to work independently and show empathy
  • Preferred: 2+ years experience in care management or with chronic illness
  • Excellent verbal communication skills
  • Familiarity with Microsoft Office and mobile/web applications
