Job summary
Meritus Medical Center in Hagerstown, MD, is seeking a Care Manager RN (Part Time) with rotating weekends and holidays. You will drive clinical coordination, guiding individuals through acute care and beyond, and work within an interdisciplinary team to assess, plan, and implement care strategies that reduce readmissions and ensure continuity of care.
The ideal candidate holds an Associate Degree in Nursing (Bachelor’s preferred), at least 2 years of acute care experience, Maryland RN license,
Qualifications
- Associate Degree in Nursing from an accredited institution required.
- Bachelor’s Degree in Nursing preferred.
- Minimum of 2 years of medical acute care experience required.
- Current Maryland RN license or Compact license with Multi-State Privilege.
- Care/case management certification within 5 years.
Responsibilities
- Coordinate patient care with the care management team during acute episodes.
- Assess patients for care coordination needs and support physician plans.
- Develop and implement discharge plans to support recovery.
- Monitor services to meet individual health needs.
- Apply evidence-based practices and utilization standards.
- Coordinate with community resources for post-discharge needs.
- Communicate across disciplines to improve clinical and financial outcomes.
- Document in EMR to support regulatory and revenue-cycle requirements.
