Job summary

Providence Medical Foundation is seeking an experienced RN to join our care management team, focusing on patient-centered, whole-person care and effective resource coordination.

The role requires California RN license and at least 3 years of clinical experience in an HMO or affiliated setting; CCM/ACM certification and related advanced degrees are preferred. You will contribute to care coordination across care teams and communities.

Qualifications

• Graduate of an accredited school of nursing.
• California Registered Nurse License upon hire.
• 3 years clinical experience in an HMO, medical group, affiliated model, hospital or medical office/clinic setting.
• Preferred: CCM or ACM certification; Bachelor’s or Master’s in related field; specialty certifications.
• 3 years experience in utilization management or case management.
