Qualifications

The Team Based Care (TBC) Manager RN independently uses the nursing process to care for a group of patients safely, therapeutically, and efficiently. This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures/protocols and documented workflows. The TBC RN collaborates with other team members to coordinate and execute the plan of care. Demonstrates initiative in the delivery of patient care in a Team Based Care Model. Actions will reflect the Providence core values.

Required Qualifications:
  • Coursework/Training: Graduate from an accredited school of Nursing.
  • Upon hire: California Registered Nurse License
  • 2 years Professional Nursing Experience.
Preferred Qualifications:
  • Bachelor's degree in nursing.
  • 1 year of experience in ambulatory care nursing.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
