Job summary

FreedomCare LLC is seeking an RN Case Manager to provide in-home assessments, education, and care coordination for patients across Atlanta and surrounding counties in Georgia.

You will develop individualized care plans, monitor patient conditions, and collaborate with physicians and caregivers to ensure high-quality, patient-centered home care. This contract role emphasizes autonomy, strong clinical judgment, and compassionate service delivery.

Qualifications

• Must graduate from an accredited nursing program.
• Active and unrestricted Registered Nurse license in the State of Georgia.
• Strong clinical assessment, communication, and documentation skills.
• Compassionate, patient-centered approach to care.

Responsibilities

• Conduct in-home clinical assessments and help patients and caregivers begin services.
• Develop, implement, and update individualized plans of care.
• Build ongoing relationships with patients and caregivers through home visits and follow-ups.
• Use clinical judgment to identify changes in condition, safety concerns, and barriers to care.
• Coordinate care with physicians, caregivers, internal clinical teams, and resources.
• Educate and coach patients on care plans, health needs, and available resources.
• Review caregiver documentation and respond to clinical changes or concerns.
• Advocate for patients to ensure safe, effective care.
• Maintain accurate clinical documentation and comply with regulatory standards.
• Collaborate with a multidisciplinary team to improve outcomes.

As an RN Case Manager, you will serve as a trusted clinical partner to patients, families, caregivers, physicians, and members of the broader care team. You will assess patient needs, develop individualized plans of care, monitor changes in condition, provide education and support, and coordinate the services and resources needed to help each patient succeed at home.

What You'll Do

• Build ongoing relationships with patients and caregivers through scheduled home visits and regular clinical follow-up.
• Use your clinical judgment to identify changes in condition, safety concerns, and barriers to successful care.
• Coordinate care with physicians, caregivers, internal clinical teams, and community resources.
• Educate and coach patients and caregivers on care plans, best practices, health and safety needs, and available resources.
• Review caregiver documentation and respond appropriately to clinical changes or concerns.
• Advocate for patients and help ensure they receive safe, effective, person-centered care.
• Maintain accurate clinical documentation and ensure care is delivered in accordance with applicable regulatory requirements and company standards.
• Collaborate with a supportive interdisciplinary team to improve patient outcomes and the overall care experience.

What We're Looking For

• Graduate of an accredited nursing program.
• Active and unrestricted Registered Nurse license in the State of Georgia
• Strong clinical assessment, communication, and documentation skills.
• A compassionate, patient-centered approach to care.
• The ability to communicate effectively with patients, families, caregivers, physicians, and other members of the care team.
• Comfort working independently while remaining closely connected to a collaborative clinical team.
• Ability to travel by car within an assigned geographic area.
• Valid driver's license and active auto insurance.
• Comfort using common clinical, case management, and productivity software.
• Experience in home care, community health, case management, or care coordination is helpful, but not required.

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay rate for this role is $120.00 per completed assessment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
