Job summary

Providence Queen of the Valley Medical Center in Napa, CA is seeking an experienced RN Case Manager to coordinate care across the continuum. This full-time role requires day shifts and emphasizes utilization management, discharge planning, and collaborative care with patients and families.

Qualifications

• Recent completion of a post-graduate level curriculum focusing on case management.

• 2 years case management, discharge planning, or utilization management in acute/sub-acute/home health or managed care.

Responsibilities

• Carry out effective case management across the continuum of care.
• Plan, coordinate, negotiate, procure and manage care for complex patients and families.
• Assist patients in utilizing appropriate health care services and potential utilization management.
• May require discharge planning and chronic disease case management in inpatient settings.

Care Manager RN at Providence Queen of the Valley Medical Center in Napa, CA

This position is Full time and will work 8-hour, Day shifts.

The RN Case Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care. The RN Case Manager assists patients in the utilization of appropriate health care services. Position may require Utilization Management and Discharge Planning, Chronic Disease Case Management based on inpatient settings.

Required Qualifications

• Coursework/Training: Recent completion of a post-graduate level curriculum focusing on case management or :


• Current clinical experience with 2 years clinical experience in an acute care hospital setting, or


• 1 year current experience in an inpatient or outpatient case management setting.


• California Registered Nurse License upon hire


• Two (2) years case management, discharge planning, or utilization management experience in the acute, sub-acute, home health setting or managed care environment.

Preferred Qualifications

• Bachelor's Degree: Nursing. Required to obtain within 3 years of hire date


• Three (3) years Acute Care Experience

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. Pay Range: $74.40 - $102.98
