Position Purpose: Develops, assesses, and facilitates complex care management activities for members with primarily physical needs to provide high-quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families.

  • Evaluates member needs, barriers to care, social determinants of health, and recommends or facilitates plans for the best outcomes.
  • Develops ongoing care/service plans and collaborates with providers to address unmet needs.
  • Identifies problems/barriers and provides appropriate interventions.
  • Coordinates between members, families, caregivers, and providers to ensure appropriate care.
  • Provides ongoing follow-up and monitors member status, updating care plans as needed.
  • Provides resource support for local services based on assessments.
  • Facilitates care management and collaborates with providers for timely access to care.
  • Performs outreach via phone, digital means, or home visits to assess needs.
  • Documents all activities to ensure compliance.
  • Educates members and families on health-related topics and service options.
  • Provides feedback to leadership for quality improvement.
  • Performs other duties as assigned.

Education/Experience: Degree from an accredited nursing school or a Bachelor's in Nursing with 2-4 years of related experience.

License/Certification: RN license required in the state of Illinois or a compact state.

Pay Range: $55,100 - $99,000 per year.
