Job description

Join to apply for the Care Manager (RN) role at RemoteWorker US.

This position is remote/work from home with 25% travel in Cook County, Illinois, supporting our YouthCare/Foster Care program. Applicants must reside in specific Illinois ZIP codes.

Position Purpose: Develops, assesses, and facilitates complex care management activities for members with primarily physical needs to provide high-quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families.

Responsibilities include:

  1. Evaluating member needs, barriers to care, and social determinants of health, focusing on member priorities.
  2. Developing ongoing care and service plans, collaborating with providers and community resources.
  3. Identifying problems/barriers to care and providing appropriate interventions.
  4. Coordinating between members, families, caregivers, and care teams to ensure appropriate, person-centered care.
  5. Providing follow-up, monitoring, and revising care plans as needed.
  6. Offering resource support for local services based on assessments.
  7. Facilitating care management and ensuring timely access to services.
  8. Conducting outreach via phone, digital means, or home visits to assess needs.
  9. Documenting all activities to ensure compliance with regulations.
  10. Educating members and families on health conditions, care gaps, and benefits.
  11. Providing feedback for quality improvement.
  12. Performing other duties as assigned.

Qualifications: Nursing degree from an accredited school or a Bachelor's in Nursing with 2-4 years of related experience. RN licensure in Illinois or a compact state is required.

Pay range: $55,100 - $99,000 annually. Centene offers comprehensive benefits including health insurance, 401K, tuition reimbursement, paid time off, and flexible work arrangements.
