Pay Range: $37 - $39 / hour. Employee benefits include health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Develop, assess, and facilitate complex care management activities for members with mental and behavioral health needs to ensure high-quality, cost-effective healthcare outcomes. This includes creating personalized care plans and providing education to members and their families about mental health and substance use disorders.
• Members require at least 1 phone call monthly, 1 visit every 90 days, and an annual assessment.
• Case managers are assigned a caseload of no more than 60 members due to the intensive nature of the members' needs.
• The purpose of the plan is to ensure members are receiving all medications, attending all MD/mental health clinic appointments, and have adequate housing and transportation to attend all appointments.

Key Requirements and Technology Experience:

• Licensed Master Social Worker (LMSW)
• Relevant years of experience
• Ability to meet strict deadlines related to documentation, visits, and calls
• Documentation must be completed within 24 hours unless on Friday
• Reports to note members out of compliance
• Ability to adapt to change
• Bachelor's degree in a health-related field; LMSW certification

Nice to have: Experience in Behavioral Health (BH).

Disqualifiers: Preference for candidates who are willing to travel.
