Care Manager Registered Nurse - Atlantic County, NJ

Join us as a Care Manager Registered Nurse at CVS Health in Atlantic County, NJ. This is a work-from-home position with 25-50% travel within a 50-mile radius of your residence.

Position Overview

As a Care Manager RN, you will serve dual eligible populations, assessing and coordinating care for members with complex health and social needs. Your role involves developing proactive care plans, collaborating with healthcare teams, and engaging members to improve health outcomes.

Responsibilities

1. Evaluate member needs using clinical tools and data review.
2. Apply clinical judgment to reduce risks and address health/social barriers.
3. Perform comprehensive assessments considering multiple data sources.
4. Refer members to clinical and community resources as needed.
5. Coordinate with healthcare providers and team members, presenting cases at interdisciplinary meetings.
6. Utilize motivational interviewing to maximize member engagement.

Minimum Qualifications

• Active RN license in NJ.
• At least 3 years of clinical practice experience.
• Reliable transportation to meet members face-to-face within the specified area; mileage reimbursed.
• Reside within 50 miles of Atlantic County, NJ.

Preferred Qualifications

• Certified Case Manager.
• 2+ years in care management, discharge planning, or home health.
• Strong communication, analytical, and problem-solving skills.
• Proficiency with computer systems and software.
• Bilingual skills preferred.

Education

Associate degree in nursing required.

Work Details

Full-time, 40 hours/week, Monday to Friday, 8 am to 5 pm. Salary range: $72,627 - $155,538, based on experience and location. Benefits include medical, 401(k), stock purchase plan, and various wellness programs.
