Purpose

We are seeking an experienced Nurse Practitioner to provide primary and palliative care to homebound patients in Central Ocean County, NJ. You will work under physician supervision, applying medical techniques and principles to care for patients with diverse chronic diseases.

Responsibilities

1. Provide high-quality care to primary and palliative patients in Ocean City, NJ.
2. Assess patient health through physical exams, medical histories, and care planning.
3. Communicate effectively with office staff and across internal platforms.
4. Deliver patient-centered care to a panel of patients.
5. Attend quarterly medical staff meetings.
6. Develop diagnoses, treatment plans, and medical impressions.
7. Build trust and consult with patients, families, and caregivers.
8. Order diagnostic tests as needed.
9. Prescribe medications, supplies, and necessary interventions.

Qualifications

• Board Certified with valid NP or PA certification in New Jersey.
• Strong clinical skills for assessment, diagnosis, and treatment.
• Positive demeanor and excellent interpersonal skills.
• Ability to work independently and problem-solve confidently.
• 1-2 years of primary care experience, preferably with geriatric or hospice patients.
