Qualifications

• At least 1 year of experience in healthcare or homecare.
• Prior experience in home care, case management, or similar roles preferred.
• Strong communication, organizational, and problem-solving skills.
• Ability to work independently and handle sensitive information with discretion.
• HIPAA compliant
• Bilingual (Spanish/English) a plus, but not required.

Responsibilities

• Conduct initial in-home assessments to determine client needs and develop personalized care plans.
• Match clients with qualified caregivers based on needs, preferences, and scheduling; provide ongoing oversight and support.
• Monitor and update care plans as needed based on client condition or feedback.
• Ensure services remain compliant with Nevada non-medical home care standards and agency policies.
• Maintain accurate client records and documentation in accordance with HIPAA and state requirements.
• Address client concerns or incidents promptly and professionally.
• Collaborate with scheduling and HR departments to ensure proper staffing coverage.
• Educate clients and families on available services and set expectations clearly.
• This role is per-diem.
