Qualifications

• Strong communication, organizational, and problem-solving skills.
• Knowledge of Nevada home care laws and regulations.
• Ability to work independently and handle sensitive information with discretion.
• Bilingual (Spanish/English) a plus.

Responsibilities

• Conduct initial in-home assessments to determine client needs and develop personalized care plans.
• Match clients with qualified caregivers based on needs, preferences, and scheduling. Provide ongoing oversight and support to both clients and caregivers, including routine check-ins.
• Monitor and update care plans as needed based on client condition or feedback.
• Ensure services remain compliant with Nevada non-medical home care standards and agency policies.
• Maintain accurate client records and documentation in accordance with HIPAA and state requirements.
• Address client concerns or incidents promptly and professionally.
• Collaborate with scheduling and HR departments to ensure proper staffing coverage.
• Educate clients and families on available services and set expectations clearly.
