Job summary

Howard Center in Vermont seeks a proactive Case Manager to work with individuals receiving medication for opioid use disorders at the Chittenden Clinic/HUB. You will carry a full caseload, provide supportive counseling and comprehensive case management tailored to each client’s needs.

You will collaborate with medical providers, nurses, and other staff to deliver person-centered care and coordinate with community partners to optimize referrals and services.

Qualifications

• Associates degree required or currently enrolled in an associates degree program.
• Apprentice Addiction Professional (AAP) certification required within 6 months of hire.
• Rostering required for pre-AAP status.
• Valid driver's license and use of a personal vehicle.
• Available for on-site interview.

Responsibilities

• Provide supportive counseling and case management.
• Partner with clinic staff to provide person-centered care to clients.
• Collaborate with community groups and treatment partners to coordinate client care and referrals.
• Provide health home services (in office) to patients as defined by the State Plan Amendment.
