Job summary

JEWISH FAMILY SERVICES OF DELAWARE INC is seeking a part‑time care management professional to assess and support older adults living in the community. You will develop strengths‑based care plans, link clients to resources, and collaborate with community providers to promote safety and independence.

You will conduct in‑home evaluations, monitor well‑being, assist families in navigating benefits, and coordinate services per the care plan.

Qualifications

• Bachelor’s degree in social work, social services, health or nursing or related field.
• Experience working with older adult populations preferred.
• Strong interpersonal skills and effective communication.
• Ability to pass a background check, including Adult Abuse Registry and driving record.

Responsibilities

• Conduct in-home evaluations to assess all aspects of the older adult’s life.
• Develop and implement personalized care plans to ensure safety and independence.
• Provide resources and referrals for support services and community partners.
• Collaborate with families to navigate benefits and options for well‑being.
• Coordinate services and conduct outreach to engage referral sources.
• Assist with all service coordination activities per the care plan.

SUMMARY:This part-time position is responsible for the provision of care management services for older adults residing in the community. Tasks include providing care management assessments and developing strengths-based care plans, as well as linking older adults and their families with community resources. You will also collaborate build relationships with community providers.RESPONSIBILITIES:Conducts in-home evaluations to assess all aspects of the older adult’s life, e.g., general health, medical providers, medications, cognitive ability, nutrition, home safety, social supports, etc.Develops and implements personalized care plans designed to assure client safety and independence as well as enhance overall well-beingResponds to client crises as neededProvides resources and referrals for other support services (e.g., in-home aides, housekeeping, financial advisor, estate planning, etc.) and recommends changes as appropriate.Conducts wellness monitoring through home visits and frequent phone contact to evaluate well-being and unmet safety concerns.Accompanies client to doctor appointments to ensure that concerns are expressed, and medical issues are understoodProvides caregiver and family consultation, including helping them navigate available resources as well as state and federal benefits, identifying appropriate options, and selecting the course of action to ensure their loved one’s well-being and their own peace of mindAssists in all service coordination activities outlined in the care planConducts community outreach designed to engage potential referral sourcesBachelor’s degree in social work, social services, human services, health or nursing or other closely related fieldExperience working with older adult populations preferredStrong interpersonal skillsAbility to pass a background check, including the Adult Abuse Registry and clean driving recordValid driver’s license, proof of auto insurance, and a clean driving recordRev 9.3.26
