Job summary

Independent Living Systems (ILS) seeks a dedicated Care Manager to coordinate comprehensive care plans for individuals needing long-term support in healthcare and social services. You will assess needs, develop discharge plans, collaborate with providers and community resources to facilitate access to Medicaid and other benefits, and monitor progress to ensure patient-centered care.

This role emphasizes compassionate support, regulatory compliance, and documentation to promote quality of life

Qualifications

• Bachelor’s degree in Social Work, Human Services, Nursing, or related field.
• Experience in case management within healthcare or social services settings.
• Knowledge of Medicaid policies and procedures.
• Relevant experience may substitute for the educational requirement on a year-for-year basis.

Responsibilities

• Assess members' physical, mental, and social needs to plan care and discharge.
• Coordinate with healthcare providers, social service agencies, and community resources to access Medicaid and benefits.
• Monitor members' progress and adjust care plans as needs change to ensure continuity of care.
• Educate members and families on services, options, and long-term care planning.
• Document cases per regulatory standards and organizational policies.
