Job summary

Molina Healthcare is seeking a Care Coordinator to support LTSS focused care management in Wisconsin Dells. You will perform comprehensive member assessments, develop and implement care plans, and coordinate services across disciplines to meet member goals.

This role requires strong communication, ability to navigate care options, and travel 25-40% to meet state contracts. A health care background with LTSS/care management experience is preferred; licensure may be required in some states.

Qualifications

• 2+ years health care experience including LTSS and care management
• Clinical licensure and/or certification if required by state contract/regulation
• Bachelor's degree may be required in some states
• Valid driver's license and reliable transportation for travel requirements

Responsibilities

• Complete comprehensive member assessments within required timelines.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care team.
• Monitor care plan effectiveness and document outcomes.
• Promote integration of behavioral health and LTSS services.
• Assess medical necessity and authorize waiver services.
• Coordinate ICT meetings for service decisions.
• Travel 25–40% as required by state contracts.
