Job summary

• Facilitate waiver enrollment/disenrollment processes.
• Develop and monitor care plans with members and providers.
• Coordinate with interdisciplinary teams to ensure quality and cost-effective care.
• Assist with identification of barriers to care and provide supportive resources.
