Job description

LOCATION: Remote - must live in or near Transylvania County, North Carolina. The person in this position is required to maintain residency in North Carolina or within 40 miles of the NC border. This position requires travel.

GENERAL STATEMENT OF JOB

The Care Manager Licensed Professional ("Care Manager - LP") is responsible for providing proactive intervention and care coordination to eligible Vaya Health members to ensure appropriate assessment and services. The Care Manager - LP collaborates with members and care teams to identify and address care gaps across mental health, substance use, intellectual/developmental disabilities, traumatic brain injury, physical health, pharmacy, LTSS, and other health-related needs. This role may include clinical transition planning and follow-up post-discharge to prevent rehospitalization. It involves work in members' communities and requires effective collaboration with various stakeholders. The Care Manager - LP utilizes clinical skills for assessments, care planning, and providing clinical guidance.

Essential functions include:

  1. Proficiency with Vaya's Care Management software and health records
  2. Outreach, engagement, and compliance with HIPAA
  3. Conducting comprehensive Health Risk Assessments (HRA)
  4. Supporting and developing person-centered care plans
  5. Reviewing clinical assessments for accuracy and providing consultation
  6. Participating in interdisciplinary team meetings and care planning
  7. Supporting transitional and diversion care efforts

Residency Requirements: Must reside in North Carolina or within 40 miles of the state border to effectively deliver in-person services.

Other Duties: As assigned.

Qualifications & Education

Master's degree in a relevant human services field required. Specific experience requirements vary based on degree and licensure, including clinical experience with mental health, LTSS, or I/DD populations. Valid licensure (RN, LCSW, LCMHC, etc.) is required and must be within scope of practice.

Salary: Commensurate with experience; position is exempt from overtime.
