Job description

LOCATION: Remote - must live in or near Transylvania County, North Carolina. The person in this position is required to maintain residency in North Carolina or within 40 miles of the NC border. This position requires travel.

GENERAL STATEMENT OF JOB

The Care Manager Licensed Professional ("Care Manager - LP") is responsible for providing proactive intervention and care coordination to eligible Vaya Health members to ensure appropriate assessment and services. The Care Manager - LP collaborates with members and care teams to identify and address care gaps across mental health, substance use, intellectual/developmental disabilities, physical health, pharmacy, LTSS, and resource networks. The role involves clinical transition planning, follow-up post-discharge, and working across various locations including members' homes. The Care Manager - LP uses licensed clinical skills to assess needs, inform care plans, and provide clinical consultation.

Essential functions include:

  1. Utilizing Vaya's Care Management software and AHR
  2. Outreach and engagement
  3. HIPAA compliance and ROI practices
  4. Conducting comprehensive Health Risk Assessments (HRA)
  5. Adhering to medication and care continuity processes
  6. Participating in interdisciplinary team meetings and care planning
  7. Managing transitional care and diversion from institutional placements

Residency in NC or near the counties served is required to effectively deliver in-person contacts.

ESSENTIAL JOB FUNCTIONS

Clinical Assessment and Care Planning

  • Identify, assess, and develop person-centered care plans
  • Link members to appropriate services across health domains
  • Conduct HRAs, administer screenings, and support care planning with clinical expertise
  • Review clinical assessments for accuracy and support providers as needed
  • Engage with members and care teams to develop and update Care Plans
  • Support crisis planning and transitional care management

Collaboration, Coordination, Documentation

  • Make independent clinical decisions supporting positive outcomes
  • Participate in multidisciplinary meetings and system barrier identification
  • Ensure documentation accuracy and compliance with policies
  • Engage in continuous training and organizational committees as needed

Other duties as assigned

KNOWLEDGE, SKILLS, & ABILITIES

  • Effective communication and interpersonal skills
  • Ability to build relationships and maintain attention to detail
  • Strong clinical assessment skills and knowledge of mental health disorders
  • Proficiency in Microsoft Office and Vaya systems
  • Understanding of NC Medicaid programs and related services

QUALIFICATIONS & EDUCATION REQUIREMENTS

Master's degree in a relevant human services field. Experience requirements vary based on degree and licensure, including specific years working with behavioral health or LTSS populations. Valid licensure (RN, LCSW, LCMHC, etc.) is required. Must meet NC Qualified Professional criteria.

PHYSICAL REQUIREMENTS

  • Visual acuity, mobility, and ability to drive
  • Ability to perform sedentary work and handle physical activity as needed

RESIDENCY REQUIREMENT: Must reside in NC or within 40 miles of the NC border.

SALARY: Commensurate with experience. Exempt position, no overtime.
