A First Name Basis (AFNB) is seeking a dedicated Care Manager in Rock Hill, SC to develop personalized care plans and supervise care staff for homebound clients. This role focuses on coordinating services and improving client well-being across our in-home care network.

The ideal candidate has 2+ years in homecare, strong communication, and organizational skills, with a valid driver’s license for local travel. Competitive pay and benefits are provided.
