Responsibilities

• Identify gaps in care and ensure continuity and quality of services.
• Monitor progress and adjust plans as needed.
• Collaborate with interdisciplinary teams to implement care plans.
• Communicate effectively with clients and providers to ensure seamless care.
• Maintain accurate records of care and progress.
• Assist in developing new processes and protocols; participate in on-call rotations.
