A First Name Basis (AFNB) is seeking a dedicated Care Manager to join our team in the in-home care field, serving seniors and individuals with disabilities. The role focuses on developing personalized care plans, coordinating resources, and supervising staff to ensure high-quality care.

The Care Manager will work to enhance client well-being through assessments, effective interventions, and collaboration with community resources, while maintaining accurate documentation and upholding

Qualifications

• 2+ years of experience in homecare preferred.
• High school diploma required; Bachelor’s degree preferred or equivalent work experience.
• Office/administrative support skills.
• Excellent written and verbal communication skills.
• Reliable vehicle for travel within area.
• Ability to work in a fast paced, multi-task environment with competing priorities.
