A First Name Basis (AFNB) is seeking a dedicated Care Manager to join our in-home care team. You will develop personalized care plans, coordinate services, and monitor client wellbeing to ensure compassionate, compliant care.

You will supervise staff, collaborate with community resources, and maintain accurate documentation in line with organizational policies. The role requires strong communication and reliable transportation.

Qualifications

• 2+ years of experience in homecare.
• High school diploma required; Bachelor’s degree preferred or equivalent work experience.
• Office/administrative support skills.
• Excellent written and verbal communication skills.
• Reliable vehicle for travel within area.
• Ability to work in a fast paced, multi-task environment with competing priorities.

Responsibilities
