Job summary

AmeriHealth Caritas Delaware is seeking a Pediatric Case Manager to support Kent County. The Care Manager will be responsible for the care coordination of our pediatric patients. The Care Manager will help coordinate care-plans and collaborate with community resources to help ensure our members receive services to their unique circumstances. The Care Manager/Coordinator (RN/SW) is responsible for engaging the member, member care giver and providers to assess plan and establish individual member goals. Will facilitate and coordinate care for the members while assuring quality and use of cost-effective resources.The position will function as a single point of contact and be an advocate for members in the care coordination program. In addition the Care Manager/Coordinator will oversee these same care management activities within assigned practices to ensure the ACDE delivers high quality care management services in accordance with Plan, NCQA, Federal/State standards and requirements.

  • Conduct telephonic and in home comprehensive assessments to determine appropriate level of care and supportive needs.
  • Provide medication management to include regular medication reconciliation and medication adherence.
  • Identify health disparities and utilize care coordinator interventions to help overcome them.
  • Up to 50% travel within Kent county will be required for the role.

Experience & Education

  • A current unrestricted Delaware LSW licensure and three (3) years professional practice experience or an active Delaware RN license.
  • A minimum of 3 to 5 years case management experience preferred.
  • A minimum of 1 to 3 years of pediatric experience preferred.
  • A valid drivers license and car insurance required
