Must reside in Hudson and/or Union County to travel for home visits within these regions. Travel up to 75% required.

Position Purpose: Develops, assesses, and facilitates complex care management activities for members with primarily mental and behavioral health needs to provide high quality, cost-effective healthcare outcomes. This includes creating personalized care plans and providing education for members and their families related to mental health and substance use disorder.

  • Evaluates the needs of the member via phone or in-home visits, related to available resources, and recommends and/or facilitates the care plan/service plan for the best outcome, including behavioral health and social determinant needs.
  • Performs telephonic, digital, home, and/or other site visits to assess member needs and collaborates with resources.
  • Develops ongoing care plans for members with high acuity and identifies providers, specialists, and community resources needed for care, including mental health and substance use disorders.
  • Coordinates between the member, family/caregivers, community resources, and care providers to ensure services are accessible.
  • Monitors care plans and member status, providing recommendations based on changes in treatment, side effects, complications, and clinical symptoms.
  • Facilitates care coordination and collaborates with providers and specialists for timely access to care and services.
  • Collects, documents, and maintains member information and care activities to ensure compliance with regulations.
  • Provides education to members and families on procedures, instructions, treatment options, referrals, and benefits, including behavioral health and social needs.
  • Offers feedback to leadership to improve care quality and delivery cost-effectively.
  • Performs other duties as assigned and complies with policies and standards.

Education/Experience: Master's degree in Behavioral Health or Social Work, or a Degree from an Accredited School of Nursing, with 2–4 years of related experience.

License/Certification:

  • Licensed Behavioral Health Professional or RN, e.g., LCSW, LMSW, LMFT, LMHC, LPC, or RN with BH experience, based on state requirements.
  • For NJ Health Plan: Master's in Behavioral Health or Social Work, with Licensed Associate Counselor (LAC) or Licensed Social Worker (LSW).

Pay Range: $55,100.00 - $99,000.00 per year.

Actual pay will be adjusted based on skills, experience, education, and other factors. Total compensation may include additional incentives.
