The Job

Contribute to the well-being of SCAN members by assessing, planning, coordinating, and monitoring their holistic needs to support aging in place. Focus on care coordination, processing referrals, and navigating healthcare services.

You Will

  1. Perform assessments and screenings to determine appropriate interventions and referrals.
  2. Serve as the initial contact point for members, providers, and medical groups for benefits administration and troubleshooting access issues.
  3. Use motivational interviewing to understand and address member goals and concerns.
  4. Coordinate care by utilizing member benefits and community resources, resolving access issues to improve health outcomes and member experience.
  5. Ensure timely and accurate documentation in multiple systems, adhering to department procedures.
  6. Apply critical thinking to resolve issues with members, providers, and vendors.
  7. Collaborate with county offices and government agencies as needed.
  8. Maintain professionalism and empathy in all interactions.
  9. Follow department procedures, workflows, and training materials.
  10. Comply with quality, regulatory, and compliance standards.
  11. Support SCAN’s vision and goals, including a curiosity about AI's role in healthcare transformation.
  12. Perform other duties as assigned.

Your Qualifications

  • High School Diploma required; some college preferred.
  • Bilingual in English and Spanish, Mandarin, Cantonese, Tagalog, Korean, or Vietnamese; skills assessed via test.
  • At least 1 year experience in healthcare or customer service.
  • Basic technical skills relevant to the role.
  • Good problem-solving, communication, and interpersonal skills.
  • Ability to work with continuous coaching and feedback.
  • Strong organizational and critical thinking skills.
  • Ability to collaborate within a multidisciplinary team.
  • Maintain confidentiality appropriately.
  • Understanding of CMS and DHCS regulations.
  • Knowledge of medical terminology and local senior resources.
