Community Healthcare Network is seeking a Health Home Plus (HH+) clinician to provide intensive case management for people living with HIV in New York City. You will conduct biopsychosocial assessments, develop patient-centered plans, and coordinate care across internal and external providers.
This role requires a Masters or BA in Human Services or related field with 1+ year of experience serving target populations; an Associates Degree with 2+ years of experience is also acceptable.

Health Home Plus (HH+) is an intensive care management program established to provide HH members the intensive services needed to stabilize their health and social service needs in the community. HH+ supports persons living with HIV (PLWH) by addressing barriers to positive health outcomes, adhering to HIV care and treatment, and achieving viral suppression.
