Monarch in Winston-Salem, NC is hiring a Care Manager Extender to support Tailored Care Management. You will work to improve health, housing stability, employment readiness, and community connections for individuals with I/DD or TBI.

Extenders assist care managers by coordinating services, documenting encounters, and helping families navigate primary care, dental, oral, and vision needs. Driving may be required.

Qualifications

  • High School Diploma required.
  • Experience navigating Medicaid delivery systems preferred.
  • Direct experience with I/DD or TBI populations is required.
  • Paid or lived experience with Tailored Care Management is required.

Responsibilities

  • Perform general outreach, engagement, and follow-up with people we support to assist individuals in making informed care and life decisions.
  • Provide a range of services to promote safety, self-worth, connection to the community, and self-advocacy.
  • Assist with goal development in the Plan of Care/ISPs and crisis plans to progress toward goals.
  • Coordinate services/appointments and arrange transportation as needed.
  • Link individuals to community-based services for housing, employment, primary care, dental and vision care.

Has two years of paid experience performing the types of functions listed in the job description, with at least one year of paid experience working directly with the Tailored Care Management eligible population.**Job Description Summary:**Extenders will support care managers in delivering Tailored Care Management by performing activities that address physical health, intellectual/developmental disability (I/DD), traumatic brain injury (TBI), long-term services and supports (LTSS), and/or pharmacy needs, in addition to unmet health-related resource needs.**What You'll Do:**• Perform general outreach, engagement, and follow-up with people we support to assist individuals to make informed choices regarding their care and services and other life decisions. • Provides a variety of services and supports to emphasize personal safety, self-worth, confidence, connection to the community, boundary setting, planning, self-advocacy, personal fulfillment, development of social supports and effective communication skills. • In conjunction with the care management team, engage in various skill-building activities such as learning how to obtain and maintain stable housing, manage finances, increase employment readiness skills, interact with others in private and public settings, maintain healthy non-violent relationships, engage in safe, non-medical ways to control modifiable risk factors that impact the person’s health (e.g. smoking, drug and alcohol use, obesity, etc.) and learn skills to increase motivation and actively address health conditions such as diabetes, heart disease, COPD, etc. • Assist in the development of the goals in the Plan of Care/Individual Support Plan and comprehensive crisis plans to assist individuals to progress to goal completion. • Coordinating services/appointments (e.g., appointment/wellness reminders, providing or arranging transportation). • Provides services in various community settings to help the individual identify and address barriers that prevent him or her from receiving needed services from healthcare professionals, including primary healthcare, dental, oral and vision. • Link individuals to appropriate community-based services to ensure that they are actively engaged in the service, including supports related to housing, and employment. • Facilitate person-centered, effective, positive relationships with people receiving services using positive approaches that promote self-determination in all areas of life. • Assist with self-help, advocacy, pre-crisis support, and supporting the individual with speaking with employers, landlords and others regarding reasonable accommodations for the person’s disability. • Document encounters and contacts made on behalf of people we support; complete and submit billing documentation. • Attend regular staff meetings and other required meetings. Convey observations and key issues to supervisor on an on-going basis. • Maintain all required training by attending and actively participating in meetings and trainings, including but not limited to, case conferences. • Maintain certification in all agency, state, and federal training requirements. • Assist new staff and/or current staff with orientation, mentoring and training. • Demonstrate knowledge of and comply with all agency policies and procedures. • Follow service definition guidelines for services being provided. • Complete all other relevant responsibilities assigned by the supervisor. • Driving and travel may be required.
