Job summary

Cross Country Healthcare is seeking a GAFC Care Manager in Southwick, MA to provide case management, psychosocial support, and care coordination for elderly and disabled GAFC members. Work closely with the RN and multidisciplinary team to ensure compliance with program requirements.

Qualifications

• 2+ years of recent experience working with elderly or disabled individuals.

Responsibilities

• Conduct initial and ongoing psychosocial assessments
• Develop, implement, and review member care plans with the RN
• Conduct required home and on-site member visits
• Provide case management and coordinate community services
• Train, evaluate, and supervise GAFC caregivers in collaboration with the RN
• Maintain accurate member records and progress documentation
• Monitor caregiver documentation and compliance
• Assist with discharge planning and referrals
• Address member housing, social, and care-related concerns
• Communicate with members, families, caregivers, building staff, and clinical teams
• Report changes in member condition to the GAFC Registered Nurse
