Job summary

Evolving Solution Services is seeking a Case Manager in Worcester, MA to oversee and coordinate services for GAFC/AFC program members, ensuring compliance and promoting safe, community-based living arrangements.

You will work with caregivers, healthcare providers, and interdisciplinary teams to support member health and independence, including assessments, care planning, and linkage to services.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, or related field.
• 1–2 years of case management or related experience preferred.
• Knowledge of GAFC/AFC regulations strongly preferred.
• Experience with elderly or disabled adults is a plus.
• Valid Massachusetts driver’s license and reliable transportation required.

Responsibilities

• Oversee and coordinate services for GAFC/AFC program members.
• Coordinate care with physicians, nurses, behavioral health providers and other services.
• Develop and maintain individualized GAFC/AFC care plans (ADL/IADL support).
• Conduct initial and ongoing assessments per regulations.
• Ensure all documentation meets compliance standards (progress notes, plans, incidents).
• Monitor caregiver performance and provide training and support.
• Collaborate with RN for clinical oversight and care plan updates.
• Respond to urgent situations and provide crisis intervention when needed.
• Participate in team meetings and quality improvement initiatives.
