The role requires a Bachelor’s degree in a health care-related field and at least one year of clinical experience. Formación

• 1 year of experience in a clinical setting, especially with Case Management principles.
• Knowledge of post-acute levels of care, community resources, public assistance systems, Medicaid or Medicare.
• BLS Basic Life Support certification from American Heart Association is required.

Responsabilidades

• Assess and allocate resources for patient management activities.
• Complete ongoing assessment of patients and families.
• Coordinate interventions with care team members.
• Evaluate effectiveness of interventions and revise care plans as necessary.
