Job summary

FreedomCare is seeking a Care Manager for our Colorado team. This field-based role requires frequent travel to patients' homes across Weld and Boulder counties, with five days of fieldwork per week in the Denver area.

You will supervise visits, support waiver members and caregivers, perform home inspections, and coordinate with the care team. A bachelor's degree or equivalent experience plus 2+ years of bedside care and 1+ year customer service are required.

Qualifications

• Bachelor's degree or equivalent work experience.
• Two+ years’ bedside care experience (CNA/QMAP/Direct Care/Medical Assistant).
• 1+ years call center, telephonic outreach, customer service or related experience.
• Strong verbal and written communication skills; ability to establish rapport with waiver members and caregivers.
• Always maintain patient confidentiality; HIPAA guidelines must be followed.

Responsibilities

• Schedule and perform quarterly Supervisory visits as required for Home Care Compliance.
• Serve as a field expert to support members and caregivers in continuing care and utilization of authorized hours.
• Perform required home inspections to ensure safety and compliance of participants’ home and surroundings.
• Monitor Daily Notes in Salesforce and document all caregiver contact.
• Maintain contact with the caregiver, virtual or in person, per service plan and case manager.
• Communicate concerns regarding member health, safety, or unmet needs to staff RN and/or Case Manager.
• Track all activities via CRM entries (Salesforce).
• Operate within a fast-paced startup healthcare environment.
• Perform other duties as required or assigned.

This is a field-based position that requires frequent travel to patient's homes across Weld and Boulder counties, CO.

FreedomCare of Colorado is home care specializing in providing home care to the Medicaid population, hiring a family member loved one to provide care. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in a home environment that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs, while preserving the dignity, self-respect, and privacy of the patient.

Working closely with waiver members, caregivers, and other stakeholders in the business, the Care Managers goal will be to provide a seamless Customer Service experience, while ensuring that our clients are receiving the care they need.

This role requires fieldwork 5 days per week throughout the Denver, CO area.

Responsibilities

• Schedule and perform quarterly Supervisory visits as required for Home Care Compliance.


• Serve as a field expert to support members and caregivers in continuing care and appropriate utilization of authorized hours.


• Perform required “home inspections” to ensure safety and compliance of participants’ home and surroundings.


• Partner with Care team to monitor Daily Notes submitted by the caregiver for indications that the caregiver may need additional training or support and document all contact with the caregiver in Salesforce.


• Maintain contact with the caregiver, virtual or in person, based on the needs identified in the service plan and calculator provided from Case Manager.


• Communicate any concerns regarding the member or caregivers’ health, surroundings, safety, or unmet needs to staff RN and/or Case Manager


• Track all activities through entries into CRM (Salesforce).


• Function within the timeframes of a fast-paced startup in the healthcare environment.


• Performs other duties as required or assigned.

Education

• A bachelor's degree or equivalent work experience.


• Two + years’ experience working bedside CNA, QMAP, Direct Care Worker, Medical Assistant, etc.


• 1+ years call center, telephonic outreach, customer service or other similar experience (required).

Qualifications

• The ability to exercise judgement independently and voice noticeable issues or trends.


• Strong verbal and written communication skills; ability to establish strong rapport to effectively communicate, guide, educate, and support waiver members and caregivers.


• Strong problem-solving skills; able to identify issues and elevate when appropriate.


• Excellent time management, critical thinking and active listening skills; able to multitask.


• Possesses a high level of empathy and emotional intelligence.


• Strong proficiency in computer software navigation; Salesforce experience, as plus.


• Comfort in a dynamic and always evolving startup environment.


• Always maintain patient confidentiality; understands and adheres to HIPAA guidelines.

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $23.00 and $26.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
