Job summary

Qualifications

• Bachelor's degree OR CASAC certification.
• Experience in case management or patient navigation.
• Proficiency with computer, telephone, fax and photocopy equipment.
• BLS certification required at hire and maintained.

Responsibilities

• Assess patients' care management needs via intake assessments.
• Maintain caseload and meet monthly core service productivity.
• Maintain electronic records and compile clinical documentation.
• Assist patients with transportation to medical and specialty appointments.
• Coordinate discharge calls and aftercare for transitions to outpatient care.
• Support pre-visit planning and communicate health status alerts to the care team.
• Advocate for patients overcoming barriers to care.
