Job summary

Family Health Centers at NYU Langone Health, a large federally qualified health center network in New York, is seeking a Care Manager to assess patient needs and manage care plans. The role involves coordinating services, advocacy, and documentation to ensure high-quality, efficient care.

Minimum qualifications include a Bachelor's degree or CASAC certification and BLS. Previous case management experience is preferred.

Qualifications

• Bachelor's degree or CASAC certification is required at minimum.
• BLS certification required at time of hire and maintained.
• Previous case management or patient navigation experience preferred.

Responsibilities

• Assess patients care management needs via patient intake assessments.
• Maintain caseload size and meet monthly core service productivity requirements.
• Maintain electronic records and complete clinical documentation within required time frames.
• Assist patient in scheduling and obtaining transportation to medical and specialty appointments.
• Conduct hospital discharge calls within 24 business hours of discharge to confirm outpatient appointments and address barriers.
• Coordinate aftercare and transition from inpatient to outpatient care.
• Conduct pre-visit planning for high risk patients and communicate health status alerts.
• Advocate for patients overcoming barriers to care (language, transportation, insurance, child care).
• Assist patients and families with benefits, entitlements, housing and other identified needs.
• Provide case management in office and via home visits with Motivational Interviewing to support behavior change goals.
