Job summary

Monarch Communities in Southbury, CT, is seeking compassionate Care Managers (Caregivers) to assist residents with daily living tasks in Assisted Living and Memory Care settings.

Under the supervision of the RN/LPN and Health and Wellness Director, you will provide personal care, meal support, hygiene, and mobility assistance, while documenting care in the EHR and upholding safety and dignity for every resident.

Qualifications

• High School Diploma or GED required.
• At least 1 year resident care experience required.
• Certified Nursing Assistant or Certified Home Health Aide certification preferred.

Responsibilities

• Assist residents with personal care, meals, bathing, dressing and mobility as per care plans.
• Administer prescribed medication under RN/LPN supervision and maintain records.
• Document services in the electronic health record and report changes to supervisor.
• Ensure safety, dignity and confidentiality of residents during shifts.
• Support residents with activities and escort to meals and programs.
• Adhere to schedules and complete all required trainings and policies.

Job Description

Led by the community's Director of Health and Wellness, we are looking for compassionate and dedicated caregivers (Care Managers) to assist residents in all aspects of their daily life as indicated in the resident's care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities.

Schedules:

• Part-timeposition(1st and 3rd shift openings) in Assisted Living and/or Memory Care
• Salary Range: $19.00 - $22.00 Hourly

Responsibilities and Duties

• Assist with self-administration of prescribed medication to resident and maintain related medical records, by way of queuing, prompting, and/or reminding; under the supervision of the RN and/or LPN.
• Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan
• Respects the needs of team members and residents by adhering to schedule and documenting attendance properly.
• Maintain the safety, comfort, dignity, and confidentiality of all residents
• Encourages and escorts (when needed) residents to meals and activities
• Ensures all assigned residents are clean, neatly dressed, groomed, safe, and fed during assigned shifts
• Document all services provided in EHR for your assigned residents during your shift
• Monitors and reports any change of condition in a resident immediately to your supervisor
• Appropriately utilizes and keeps electronic documentation "devices" safe and secure within the community; efficiently documents care/services provided during the shift.
• Participates in and attends all in-service trainings as scheduled
• Ensures compliance with all Federal, State, and local regulations, as well as community policies and procedures
• Provides basic housekeeping and/or laundry services when needed
• May be asked to obtain weights and vitals of residents and document same per RN request
• Other duties as assigned by supervisor, RN, and/or ED

Qualifications

• High School Diploma or GED. At least 1 year resident care experience required.
• Certified Nursing Assistant or Certified Home Health Aid
• Knowledge and experience in working with the elderly population, preferred dementia experience

Physical Abilities

• While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
• An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
