Job description

NOTE: This is a remote telephonic role that offers care management services to members diagnosed with chronic kidney disease. Preference will be given to applicants who are (1) based in Florida, and (2) have professional experience with chronic kidney disease, behavioral health, and case management.

Schedule: 8-5 PM ET Zone

Position Purpose:

Develops, assesses, and facilitates complex care management activities for members with mental and behavioral health needs to provide high-quality, cost-effective healthcare outcomes. This includes creating personalized care plans and providing education to members and their families related to mental health and substance use disorders.

1. Evaluate member needs via phone or in-home visits, considering available resources, and recommend or facilitate care plans that may include behavioral health and social determinants.
2. Perform outreach through telephonic, digital, home, or other site visits to assess needs and collaborate with resources.
3. Develop ongoing care plans for members with high acuity, identifying providers, specialists, and community resources needed for comprehensive care.
4. Coordinate between members, families/caregivers, community resources, and care providers to ensure access to services.
5. Monitor care plans and member status, noting changes in symptoms or treatment side effects, and adjust plans accordingly.
6. Facilitate care coordination to ensure timely access to necessary services.
7. Document and maintain member information and care activities in compliance with regulations.
8. Educate members and families on procedures, healthcare instructions, treatment options, referrals, and benefits, including behavioral health and social needs.
9. Provide feedback to leadership to improve care quality and delivery cost-effectively.
10. Perform other duties as assigned and ensure compliance with policies and standards.

Education/Experience:

Requires a Master's degree in Behavioral Health, Social Work, or Nursing from an accredited school, with 2-4 years of related experience.

License/Certification:

• Licensed Behavioral Health Professional or RN, e.g., LCSW, LMSW, LMFT, LMHC, LPC, or RN with behavioral health experience, as required by state contract.

Pay Range: $55,100.00 - $99,000.00 per year

Actual pay will be adjusted based on skills, experience, education, and other factors. Total compensation may include additional incentives.
