This job is located in the Bronx with travel to Brooklyn office

Purpose of the Position

The Care Managers service youth enrolled in Medicaid, with two or more chronic conditions, HIV/AIDS, Sickle Cell, Serious Emotional Disturbance (SED), Serious Mental Illness (SMI), or Complex Trauma, ages 0-21 (Children’s Program) and 18 to end of life (Adult Program). The Care Manager plays a critical role in coordinating, integrating, and partnering in the delivery of care to children, adults, and families with complex needs.

Responsibilities
  1. Obtain required Care Management enrollment consents from the medical consenter.
  2. Complete initial and ongoing needs assessments such as the Comprehensive Assessment, CANS-NY, and NYS Eligibility Assessment for HARP.
  3. Develop and manage the Plan of Care (POC), coordinating services, supporting treatment adherence, and facilitating referrals.
  4. Monitor and evaluate needs related to prevention, wellness, medical, mental health, care transitions, and social services.
  5. Meet billing requirements based on acuity levels, managing caseloads that may include high, medium, and low acuity members, both children and adults.
  6. Provide required face-to-face contacts as per Department of Health mandates.
  7. Ensure timely and accurate documentation using activity logs and electronic health records.
  8. Advocate for members within the agency and with external providers.
  9. Promote wellness by linking members to resources and educating them on health conditions and preventative care.
  10. Assist members in obtaining public benefits such as Medicaid, SNAP, housing, legal services, employment, and training supports.
  11. Communicate effectively with members and families, considering language, literacy, and cultural factors.
  12. Conduct care planning meetings and coordinate services with interdisciplinary teams.
  13. Manage referrals and engagement with community resources.
  14. Participate in discharge planning for hospital admissions.
  15. Attend ongoing staff development training.
  16. Ensure periodic evaluations and follow-ups for dental, vision, and hearing care per Medicaid EPSDT guidelines.
  17. Develop close working relationships with case planners and managers.
  18. Travel within five boroughs and Nassau and Suffolk counties.
  19. Perform other duties as assigned.
Qualifications
  • Bilingual preferred.
  • For high-acuity children or adults with Health Home Plus status: a Bachelor’s degree with two years’ relevant experience, or RN license with two years’ experience, or Master’s degree with one year’s experience.
  • For medium-acuity children with SED: at least an associate’s degree with one year’s relevant experience.
  • Experience in servicing youth, adults, and families in child welfare, developmental disabilities, mental health, or healthcare systems.
  • Passion for advocacy and strong communication, leadership, and crisis intervention skills.
