This job is located in the Bronx with travel to Brooklyn office
The Care Managers service youth enrolled in Medicaid, with two or more chronic conditions, HIV/AIDS, Sickle Cell, Serious Emotional Disturbance (SED), Serious Mental Illness (SMI), or Complex Trauma, ages 0-21 (Children’s Program) and 18 to end of life (Adult Program). The Care Manager plays a critical role in coordinating, integrating, and partnering in the delivery of care to children, adults, and families with complex needs.
Preferred Education: Bachelor’s degree
