Job summary

Jobtailor is seeking a Care Management Assistant in a patient-focused role that collaborates with nurses and social work care managers to optimize patient care. You will work in a clinical setting in Ogden, UT and support care teams to ensure seamless care coordination.

Responsibilities include coordinating appointments, facilitating communication with physicians and families, educating patients about resources and regulatory or payer notices, monitoring referrals, and helping navigate community

Qualifications

• Healthcare experience in a clinic or hospital setting.
• Customer service with a focus on communications and problem resolution.
• Proficiency in advanced computer skills.
• Caregivers whose duties require them to conduct home or community visits must maintain current BLS certification, have a current driver’s license, current auto insurance, an acceptable driving record and reliable transportation.
• Associate or bachelor’s degree preferred.
• Previous experience with medical terminology.
• Excellent verbal and written communications skills.
