A First Name Basis (AFNB) is expanding its team and seeks a dedicated Care Manager to develop personalized care plans and coordinate services for clients in home-based settings. This role involves supervising staff, accessing community resources, and ensuring quality care and compliance across our aging population.

The Care Manager will assess client needs, implement interventions, and maintain thorough documentation, contributing to improved client well-being and satisfaction.

Qualifications

• 2+ years of experience in homecare preferred.
• High school diploma required; Bachelor’s degree preferred or equivalent work experience.
• Office/administrative support skills.
• Excellent written and verbal communication skills.
• Reliable vehicle for travel within area.
• Ability to work in a fast paced, multi-task environment with competing priorities
