A First Name Basis (AFNB) is a rapidly growing in-home care provider with 40+ offices across four states, seeking a dedicated Care Manager to develop personalized plans and supervise staff.

The role emphasizes coordinating services, documenting client progress, and collaborating with community resources to ensure high-quality care for seniors and individuals with disabilities.

Qualifications

• 2+ years of homecare experience.
• High school diploma required; Bachelor’s degree preferred or equivalent work experience.
• Strong written and verbal communication skills.
• Reliable vehicle for travel within area.
• Ability to work in a fast paced, multi-task environment with competing priorities.

Responsibilities

• Develop and implement individualized care plans tailored to the unique needs of each client.
• Supervise and mentor staff members, fostering a collaborative environment that promotes professional growth.

Position Summary:
