Job summary

Person Centered Services is seeking a Social Work Professional for Care Coordination in a health home model serving individuals with intellectual and developmental disabilities. The Care Coordinator, also known as a Care Manager, coordinates health, behavioral healthcare, community supports, and other services to meet each person’s needs.

The role emphasizes a strengths-based, person-centered approach with collaboration among health care providers and support teams.

Qualifications

• Bachelor's degree with 2 years of relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master's degree with 1 year of relevant experience.
• Minimum Bachelor’s degree required by OPWDD in NYS.
• Relevant experience consists of providing direct care or support, coordination, and/or advocacy for anyone (e.g., Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Manager).
• Valid driver’s license is required.

Responsibilities

• Develops a comprehensive, person-centered Life Plan with the individual and their circle of support, and with the entire service provider team.
• Manage a tiered caseload, tailoring services to individual needs, similar to a Care Manager.
• Completes enrollment and eligibility documentation.
• Meets individuals in their homes, physician/provider offices, and other public places to conduct assessments and provide services.
• Commit to a respectful, just, and supportive environment aligning with diversity, equity, and inclusion.

Person Centered Services Care Coordination Organization is looking for a passionate Social Work Professional to work within a health home model serving people with intellectual and development disabilities. The Care Coordinator also known as a Care Manager has an overall responsibility for coordinating and advocating for all aspects of the individual's care including, but not limited to, health and behavioral healthcare, community supports, and other services required to meet the needs of the individual. Care Coordinators and Social Workers use a person-centered, holistic approach, where each person being served has their health care and other service providers collaborate to ensure their wants and needs are met. We're in the business of helping people and always using a strengths-based approach.
