Job summary

Person Centered Services Care Coordination Organization, LLC is seeking a Social Work professional to coordinate and advocate across health, behavioral healthcare, and community supports for individuals with intellectual and developmental disabilities.

The Care Coordinator, also known as Care Manager, will engage with individuals in their homes and in the community, ensuring holistic, person-centered care and collaboration among care providers. Prior DPS/CARE roles are welcomed.

Qualifications

• Bachelor's degree with 2 years of relevant experience or RN with 2 years of relevant experience or Master's with 1 year of relevant experience.
• OPWDD NYS requires at least a Bachelor's degree.
• Experience in direct care, coordination, and/or advocacy (e.g., Direct Support Professional, Social Worker, Care Manager).
• Valid driver’s license is required.

Responsibilities

• Develops a comprehensive, person-centered Life Plan with the individual and their support circle.
• Effectively manage a tiered caseload and tailor services to individual needs.
• Completes enrollment and eligibility documentation.
• Meets with individuals in homes or community settings to conduct assessments and provide services.
• Upholds a respectful, inclusive environment aligned with the company’s diversity and inclusion commitments.

New Employee Orientation (NEO)

• All new team members participate in NEO offered Monday - Friday both onsite and online.
• Day one includes in-person training at our West Seneca, NY office, where new team members are introduced to our culture, values, and the foundations of Care Coordination.

Hybrid Schedule

• Care Coordinators are required to attend three onsite days per week during the first 90 days to support on boarding and integration
• After successfully completing this period, team members may transition to a hybrid schedule for added flexibility. Supervisors may extend the on-site requirement if needed.

Person Centered Services Care Coordination Organization is looking for a passionate Social Work Professional to work within a health home model serving people with intellectual and development disabilities. The Care Coordinator also known as a Care Manager has an overall responsibility for coordinating and advocating for all aspects of the individual’s care including, but not limited to, health and behavioral healthcare, community supports, and other services required to meet the needs of the individual. Care Coordinators and Social Workers use a person-centered, holistic approach, where each person being served has their health care and other service providers collaborate to ensure their wants and needs are met. We’re in the business of helping people and always using a strengths-based approach.

Social workers, care managers, and case managers are encouraged to apply! If you have worked as a Direct Support Professional this may be the right fit for you.

Position Responsibilities

• Develops a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team.
• Effectively manage a tiered caseload, while tailoring services to individual needs. Similar responsibilities as a Care Manager.
• Completes enrollment and eligibility documentation.
• Meets with individuals in their homes, physician/provider offices, and other public places to conduct assessments and provide services.
• Commits to a respectful, just, and supportive environment for individuals and coworkers aligning with the company’s commitment to diversity, equity, and inclusion.

Requirements

• Bachelor's degree with 2 years of relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master's degree with 1 year of relevant experience
• Minimum of a Bachelor's degree is required by OPWDD in NYS
• Relevant experience consists of providing direct care or support, coordination, and/or advocacy for anyone. Examples: Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Manager, etc.
• Valid driver's license
