Job summary

Entyre Care is seeking a dedicated Home Health Case Manager in New Hampshire to conduct home visits, assess clients, and implement care plans for elders and adults with disabilities. Qualifications

  • Bachelor’s degree required in social work, human services, or related field
  • At least 1 year of experience with elders or adults with disabilities
  • Ability to travel by automobile; valid driver’s license and auto insurance
  • Experience in case management, care implementation, planning and assessment for home care
  • Training, evaluating, and supervising caregivers
  • Using teaching principles to counsel patient, family and caregiver
  • Documenting accurately and timely; maintain clinical records
  • Proficiency in MS Office and web-based applications (EVV, etc.)
  • Patient-centered approach with strong communication skills
  • Familiarity with NH Medicaid, CFI and caregiving ecosystem preferred

This role requires frequent travel. You're expected to drive for several hours each day, normally conducting 3-5 home visits (dependent on travel times).

Key Responsibilities

What You Will Bring (Requirements)
  • Bachelor’s degree required; preferably in social work, human services, or a related field.

    • LSW or LISW in state of employment preferred.

  • At least one (1) year of experience working with elders or adults with disabilities.

  • Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required.

  • Experience in case management, care implementation, planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting.

  • Training, evaluating, and supervising caregivers.

  • Using basic teaching principles, counsel and instruct the patient, family and caregiver in meeting their care related needs.

  • Documenting in an accurate and timely manner; maintain a clinical record for each patient.

  • Proficiency in common software and web-based applications (e.g. MS Office, case management systems, EVV, etc.).

  • Patient-centered approach with strong communication skills.

  • Familiarity with New Hampshire Medicaid, CFI and/or caregiving ecosystem (including CMAs, government offices, and other stakeholders) is preferred.
