Job summary

Entyre Care is seeking a full-time hybrid leader to support caregivers and coordinate outpatient home-care services. You will conduct home visits, develop care plans, and train caregivers to meet clients’ needs across aging and disability contexts.

The role emphasizes travel, collaboration with physicians and community resources, and compliance with standards. A Bachelor’s degree is required, with LS/W LISW preferred and 1+ year of relevant experience.

Qualifications

• Bachelor’s degree required; preferably in social work, human services, or a related field.
• LSW or LISW in state of employment preferred.
• At least one (1) year of experience working with elders or adults with disabilities.
• Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required.
• Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting.
• Training, evaluating, and supervising caregivers.
• Using basic teaching principles, counsel and instruct the patient, family and caregiver in meeting their care related needs.
• Documenting in an accurate and timely manner; maintain a clinical record for each morbidity patients.
• Monitoring the health status of all members to ensure that all needed care is properly delivered.
• Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc).
• Additional state-specific qualifications may be required.

This role requires frequent travel. You're expected to drive for several hours each day, normally conducting 3-5 home visits (dependent on travel times).
