Job summary

Spectraforce Technologies, Inc. is seeking a Care Manager for a remote Florida-based assignment lasting 3 months. The role focuses on improving care coordination and reducing readmissions.

Maintain a caseload of Behavioral Health Complex members; outreach to providers; develop care plans and update EMR; collaborate with community resources.

Requires a Master’s in Behavioral Health or Social Work or Nursing and an active Florida BH license; schedule is M-F 8-5 EST; remote arrangement.

Qualifications

• Requires a Master's degree in Behavioral Health or Social Work or a degree from an Accredited School of Nursing.
• 2–4 years of related experience is required.
• Must hold an active Florida Behavioral Health License (LMHC, LCSW, LMFT).

Responsibilities

• Maintain an active caseload of about 73 Behavioral Health Complex members at a time.
• Outreach to member, primary care physician and outpatient providers biweekly.
• Initiate appropriate assessment using our internal EMR system.
• Create a Care plan tailored to the member's needs.
• Update Medications, diagnosis, and allergies in EMR monthly.
• Review care gaps and offer education and resources to close gaps.
• Connect member to providers and resources in the community.
• Attend scheduled community staffings and treatment teams with community agencies.
• Staff complex members as needed with internal medical directors and clinical staff.

Position Summary:

Our team focuses on enhancing the quality of services we provide to our members while also reaching as many members as possible. This team has an extremely high volume of referrals for members needing care management. This role will directly impact the goals listed below by driving member to use healthcare services appropriately and guiding them towards supports in the community.

• Prevent admissions/reduce readmissions
• Close care gaps via encouraging member to get annual screenings, checkups, and adhere to suggested treatment and/or medication regimens.

Key Responsibilities:

• Maintaining an active caseload of about 73 Behavioral Health Complex members at a time.
• Outreaching member, primary care physician and outpatient providers on a biweekly basis
• Initiating appropriate assessment using our internal EMR system
• Creating a Care plan tailored to the member's needs.
• Updating Medications, diagnosis, and allergies in our EMR system on a minimum of a monthly basis.
• Reviewing care gaps and offer education and resources in efforts to close those care gaps.
• Connecting member to providers and resources in the community.
• Attending scheduled community staffings and treatment teams as well as partnering with community agencies.
• Staff complex members as needed with internal medical directors and clinical staff.

Qualification & Experience:

Education/Certification Required: Requires a Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing and 2 - 4 years of related experience. Preferred:

Licensure Required: Must have an active Florida Behavioral Health License (LMHC< LCSW< LMFT)
