Job summary

St. Francis Hospital & Heart Center® in Roslyn, NY, is seeking a Care Manager to lead individualized care planning for patients across the acute care continuum. You will assess needs, coordinate services, and collaborate with the care team to ensure optimal outcomes.

The ideal candidate has NYS RN licensure, a BSN, 3–5 years of med/surg or cardiac experience, and BLS certification. PRI or case management certification is preferred, with a commitment to compassionate, patient-centered care.

Assesses, plans, implements, monitors and evaluates options and services to effect an appropriate individualized plan of care for patients across the acute care continuum using independent judgment and discretion.

Requirement

NYS RN License

B.S. in Nursing3-5 yrs.

BLS certified with the American Heart Association

Recent med/surg or cardiac experience, PRI and/or case management certification preferred.

Salary Range

USD $49.65 - USD $74.76 /Hr.
