About the Role
We are seeking a compassionate and dedicated Care Manager to join our Care Management team supporting individuals with Intellectual and Developmental Disabilities (I/DD). In this role, you will coordinate services, advocate for members, and ensure each individual receives comprehensive, person-centered care that supports their goals, independence, health, and quality of life.
This position involves a combination of field visits, regional travel, and office-based responsibilities. It is not a fully remote position.
Key Responsibilities
- Provide person-centered care management in accordance with regulatory standards and organizational policies.
- Complete comprehensive assessments and reassessments.
- Identify service gaps and coordinate appropriate referrals.
- Advocate for members to achieve their personal goals and maintain quality of life.
- Develop and maintain individualized Life Plans within required timelines.
- Lead interdisciplinary team meetings and coordinate services.
- Resolve conflicts and barriers within the planning process.
- Ensure accurate and timely documentation, including billing compliance.
- Maintain member eligibility through annual Level of Care Determinations and HCBS waiver processes.
- Assist members with accessing Medicaid, Social Security, SNAP, and other benefits.
- Maintain accurate electronic health records and member consents.
- Coordinate medical, behavioral health, community, and specialized services.
- Follow up on critical healthcare needs and support preventive care initiatives.
- Assist with transitions between healthcare settings and adult services.
- Utilize electronic health records (EHR) systems and telehealth technology.
- Attend required trainings, meetings, and supervision sessions.
- Travel throughout the service area to meet with members and providers.
- Follow incident reporting procedures and ensure member safety.
- Maintain HIPAA compliance and confidentiality standards.
- Perform additional duties as assigned.
Qualifications Required
- Bachelor's Degree and at least 2 years of relevant experience, OR
- Registered Nurse (RN) license with 2 years of relevant experience, OR
- Master's Degree with 1 year of relevant experience.
